Clinical Study on Non-union of Fractures of Long Bones



Abstract
Non-union is one of the most serious complication of the fractures of the long bone, and it is often difficult to achieve union.
Forty one cases with non-union of the long bones who had been treated by rigid stabilization and cancellous bone grafting at the Department of Orthopedic Surgery, Chonbuk National University Hospital, from March, 1980 to December, 1984 are clinically analysed.
The results are as follows ;
1 . Of the 41 cases, there were 38 male and 3 female patients.
2 . Non-union more commonly occurred in active aged person. 33 cases (79.6%) were in third, fourth, and fifth decade.
3 . The most common cause of injury was traffic accident(73. 2% ).
4 . Of the 41 cases, there were 21 closed and 20 open fractures. The most commonly involved bone was tibia(25 cases : 61% ).
S. As the initial treatment of fractures of the long bone, operative treatment was done in 33 and conservative in 8 cases.
6 . The causes of non-union were infection in 22, unsatisfactory immobilization in 11, and implant failure in 8 cases.
7 . The most common causative organism of infected non-union was Staphlococcus aureus.
8 . By rigid osteosynthesis and cancellous bone grafting, bony union was achieved in average 6.2 months in all of the 19 cases of noninfected non-union, and in average 7.4 months in 16 of the 22 cases of infected non-union.
